Decatur County Memorial Hospital is hosting a Sports Physical Night for area student-athletes. Visit Tree City Medical Partners on June 6 from 6-8 p.m. Physicals are first-come, first-serve and are $20, cash or check only. A parent or guardian MUST be present. Please fill out the Pre-Participation Physical Evaluation Form (PPE), found here, and bring it with you.


Sports Physicals are for student-athletes with no pre-existing conditions. If you are currently seeing a physician or taking prescription medications for an acute illness, please have your physical completed by your primary care physician.
